The following forms are to be filled out for all B'nei Mitzvah and returned to the appropriate staff. 

Honors Form (to be returned to the Clergy Assistant the week prior to B'nei Mitzvah Date)

Participation Form (to be returned to the Executive Director - 30 days prior to B'nei Mitzvah date)
